This package of forms and associated scripts and standard operating procedures for client tracing is from the Assisted Partner Services Scale-up implementation trial in western Kenya. The trial is being conducted by investigators at the University of Washington in partnership with PATH and the Kenyan Ministry of Health.

  1. HIV Testing and Counseling Intake Form
    This intake form is to be completed by HIV testing services providers for all people approached for an HIV test. It has sections for recording information about the client and the HIV testing services received, including test results and linkage/referral to HIV care or other services, as appropriate. Client information includes sociodemographic characteristics, contact information, HIV testing and treatment history, whether the client sought services in response to partner notification, and HIV-associated risk behaviors.
  2. Assisted Partner Notification Willingness Form
    This form is to be completed with clients who test HIV-positive to assess their willingness to participate in assisted partner notification services. It documents the offer of assisted partner notification services, whether a client consents to receive these services, and (if necessary) reasons for refusal.
  3. Assisted Partner Notification Intervention Screening Form
    This screening form is to be completed with clients who consent to participate in assisted partner notification services. It is used to assess clients’ risk of intimate partner violence to determine eligibility for assisted partner notification services and identify clients in need of monitoring for intimate partner violence. Clients at high risk for violence are not eligible for assisted partner notification services, and those at moderate at risk are eligible but referred for additional monitoring.
  4. Sexual History with Partners Form
    This form is used to record information about the partner’s relationship with the index client, sociodemographic information, and HIV status as well as the index client’s preferred method for notifying this partner.
  5. Partner Contact Form
    The Partner Contact form is used to record contact information for each of the index client’s sexual partners.
  6. Client Tracing Form
    This form is used for each elicited partner to track attempts to contact the partner and record outcomes of partner notification.
  7. Scripts for Phone and In-Person Tracing
    These scripts are used by partner notification services providers when calling or conducting an in-person visit with partners. They describe how providers should confirm that they have reached the correct person, inform the partner of potential HIV exposure, offer HIV testing, and introduce assisted partner notification services.
  8. Client Tracing Standard Operating Procedure
    This standard operating procedure was developed to guide partner notification services providers through the process of tracing contacts for partner notification and documenting this process. It is meant to be used with the forms and scripts described above.
