Partner Notification Tracing and Testing (PNTT) Initial Assessment Form

This form, written in Khmer and English, is used by the National Center for HIV/AIDS, Dermatology and STDs (NCHADS) to record results of an initial partner notification services assessment for adults living with HIV, and follow-up with their partners and children who need HIV testing. It includes sections on (1) listing each elicited partner or child along with their basic HIV status and care information; (2) recording relationship information and choice of partner notification strategy for each partner; (3) tracking notification, tracing, and HIV testing outcomes for each partner; and (4) assessing adverse events. It can be used for general and key populations, including men who have sex with men, trans people, and persons who use or inject drugs.
